Remedy Social Care is seeking an Adult Social Worker to join the Hospital Team in Sefton. You will assess adults in hospital, coordinate care plans, and support safe discharge, working with families and professionals to address social care, safeguarding and wellbeing needs.
You will promote independence and person-centred care, participate in MDT meetings and carry out safeguarding duties, ensuring records are accurate and compliant with policies.
