Our client Rhondda Cynon Taf County Borough Council is looking for a Qualified social worker to join their Mental health team.

Duties:

  • Work as part of a team and understand the strengths and challenges of multi-disciplinary working to deliver an effective and efficient service to the public.
  • Safeguard and promote the wellbeing of adults through high-quality social work practice in assessment, care planning, and review.
  • Co-produce and implement care and treatment plans and act as a care co-ordinator in accordance with key legislative requirements of the Mental Health Measures Wales (2010).
  • Work within and understand the principles of the Social Services and Well-being (Wales) Act (2014).
  • Undertake mental capacity assessments and work within the principles of the Mental Capacity Act 2005.
  • Be familiar with WARRN risk assessments and assess risk in a collaborative and structured way that balances risk with autonomy and choice.
  • Identify and promote individual strengths and independence.
  • Work in collaboration with other agencies and adopt a multi-disciplinary approach where appropriate.
  • Ensure that electronically held records, including case recordings, assessment, care and treatment plans, and reviews are completed and updated in accordance with departmental policy and procedures.
