Our client Cambridgeshire county council is looking for an Adults Social Worker to join their Learning Disabilities team.
Key Responsibilities
- Complete Care Act 2014 assessments, including complex and high-cost care packages
- Undertake timely reviews following changes in individuals' needs or circumstances
- Deliver a minimum of two completed and recorded assessments per week
- Carry out face-to-face assessments and reviews
- Actively involve advocates, carers, and families
- Analyse and evaluate care and support arrangements
- Work closely with brokerage and contracts colleagues
- Ensure care packages maximise independence and wellbeing
- Maintain accurate and defensible records
Essential Requirements
- Qualified Social Worker with Social Work England registration
- Experience with Learning Disabilities/Autism
- Knowledge of Care Act 2014 and Mental Capacity Act
- Experience reviewing complex care packages
- Access to own vehicle is essential
