Our client Blackburn with Darwen Council is looking for an Adults Social Worker to join their Hospital/Discharge to assess Team.
We are looking for a skilled and motivated Social Worker to join our Hospital Discharge and Discharge to Assess (D2A) service. This is a dynamic, fast-paced role focused on supporting timely, safe, and person-centred discharges from hospital into community settings via Discharge to Assess pathways.
Working as part of a multi-disciplinary team, you will play a key role in ensuring individuals leave hospital with the right support in place, enabling them to recover and regain independence in the most appropriate environment. This is a hybrid role, offering a balance of office based, on-site hospital work, community visits, and remote working.

Key Responsibilities
- Undertake strengths-based assessments in line with Care Act principles
- Facilitate safe and timely hospital discharges through the D2A pathway
- Work collaboratively with health professionals, families, and community services
- Promote independence, wellbeing, and risk enablement
- Develop and review support plans that reflect individual goals and outcomes
- Safeguard vulnerable adults and manage risk appropriately
- Maintain accurate and timely case records
About You
- Qualified Social Worker with current Social Work England registration
- Experience in adult social care, ideally within hospital discharge or community settings
- Strong understanding of strengths-based and person-centred practice
- Ability to work in a fast-paced environment and manage competing priorities
- Excellent communication and partnership-working skills
- Confident decision-maker with a proactive and solution-focused approach
