Our client Blackburn with Darwen Council is looking for an Adults Social Worker to join their Hospital/Discharge to assess Team. The role focuses on supporting timely, safe, and person‑centred discharges from hospital into community settings via Discharge to Assess pathways.
Our practice is grounded in person‑centred, strengths‑based principles. We work collaboratively with individuals, families, and partner agencies to build on existing strengths, promote independence, and achieve meaningful outcomes. We prioritise what matters to each person, ensuring their voice is central to all decision‑making.
Key responsibilities- Undertake strengths‑based assessments in line with Care Act principles
- Facilitate safe and timely hospital discharges through the D2A pathway
- Work collaboratively with health professionals, families, and community services
- Promote independence, wellbeing, and risk enablement
- Develop and review support plans that reflect individual goals and outcomes
- Safeguard vulnerable adults and manage risk appropriately
- Maintain accurate and timely case records
- Qualified Social Worker with current Social Work England registration
- Experience in adult social care, ideally within hospital discharge or community settings
- Strong understanding of strengths‑based and person‑centred practice
- Ability to work in a fast‑paced environment and manage competing priorities
- Excellent communication and partnership‑working skills
