Our client Newcastle City Council is looking for an Adult Social Worker to join their Learning disabilities team.

Job Purpose

To provide high-quality social work services to adults with learning disabilities, promoting independence, wellbeing, choice, and inclusion in accordance with the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation. The post holder will assess needs, develop support plans, manage risk, and work collaboratively with individuals, families, carers, and partner agencies.

Key Responsibilities

  • Complete strengths-based assessments under the Care Act.
  • Develop, implement, and review person-centred care and support plans.
  • Undertake safeguarding enquiries and investigations in line with local safeguarding procedures.
  • Assess and manage risks while promoting positive risk-taking and independence.
  • Complete Mental Capacity Assessments and Best Interests decisions where appropriate.
  • Support individuals through transitions, hospital discharge, and community integration.
  • Arrange and review commissioned care packages and direct payments.
  • Work collaboratively with health professionals, housing providers, advocacy services, and voluntary organisations.
  • Maintain accurate, timely, and legally compliant case records.
  • Manage a complex caseload, prioritising work according to risk and statutory responsibilities.
  • Prepare reports for funding panels, legal proceedings, and safeguarding meetings where required.
