Our client Swindon Council is looking for a Social Worker to join their Learning Disabilities Team.
Job Purpose To provide high-quality, person-centred social work services to adults with learning disabilities, promoting independence, wellbeing, choice, and inclusion.
The post holder will assess eligible needs, develop outcome-focused support plans, undertake safeguarding responsibilities, and work collaboratively with individuals, families, carers, and partner agencies to achieve positive outcomes in accordance with statutory duties.
Key Responsibilities Manage a caseload of adults with learning disabilities, ensuring timely assessment, support planning, and review.
Undertake strengths-based assessments under the Care Act 2014, identifying eligible needs and promoting independence.
Complete Mental Capacity Act (MCA) assessments and Best Interests decision-making where appropriate.
Undertake safeguarding enquiries in line with Section 42 of the Care Act and local safeguarding procedures.
Develop person-centred care and support plans that maximise independence and community participation.
Carry out risk assessments and implement effective risk management plans while promoting positive risk-taking.
Work in partnership with individuals, families, carers, advocates, health professionals, housing providers, and voluntary organisations.
