Key Responsibilities
- Carry out holistic assessments of children and young people with disabilities in their home environments.
- Assess, recommend and provide specialist equipment to support daily living and independence i.e. specialist seating, moving and handling, bathing and toileting.
- Assess for and contribute to home adaptations, ensuring environments are safe, functional, and fit for purpose.
- Work closely with families and carers, providing guidance, reassurance, and practical support.
- Collaborate with multi-disciplinary professionals across Council services, Health (e.g., NHS colleagues), Education providers, and partner and voluntary agencies.
- Contribute to care planning, ensuring interventions are child-centred and outcome-focused.
