Our client Warrington Borough council is looking for an Adults Social Worker to join their Hospital Team. We are seeking 2 experienced agency Social Workers to join the Transfer of Care Hub at Warrington hospital. This is a fast-paced, hybrid role supporting safe and timely discharges from hospital.
The post is full time, 37 hours per week, with a requirement to base in the office around 3 days per week. Recent experience in hospital or Transfer of Care setting is essential, along with strong assessment, communication and decision making skills. Knowledge of social care system Mosaic is desirable.
Referral bonus - up to £250* per person placed.
You will be required to fully register with Remedy for all locum positions. Remedy Social Care is an APSCo Standard agency and is committed to safeguarding and promoting the welfare of the vulnerable. Successful applicants will be required to undertake an Enhanced Disclosure via the DBS.
