Role Purpose:
The Referral Coordinator at Rehability UK will manage the full referral and assessment process across Learning Disability, Autism, and primarily Mental Health services. The postholder will ensure referrals are appropriate, progressed efficiently, and matched to the most suitable service, property, and care model. The role requires strong relationship management skills, KPI focus, and the ability to confidently present and promote Rehability UK’s services to internal managers and external stakeholders.
Key Responsibilities:
- Referral & Assessment Management: Manage the full end-to-end referral process from initial enquiry through to assessment and placement outcome. Review referrals to ensure they are relevant, complete, and suitable for Group’s services. Coordinate and schedule assessments, managing diaries and ensuring agreed timescales are met. Work closely with Service Managers, Registered Managers, and clinical professionals during the assessment process.
- Relationship & Stakeholder Management: Develop and maintain strong working relationships with internal managers and external services. Act as a key point of contact to ensure referrals and assessments are aligned to service capability and capacity. Support managers in making informed placement decisions.
- Service Promotion & Placement Matching: Confidently pitch Group’s services, properties, and care models to commissioners and referrers. Match referrals appropriately based on needs, risks, and service availability. Provide clear and accurate service information to support placement decisions.
- KPI, Diary & Performance Management: Manage assessment diaries, follow-ups, and referral tracking. Work to agreed KPIs including referral turnaround, assessment completion, and placement conversion. Maintain accurate records and produce referral activity reports as required.
- Quality & Compliance: Ensure all referral and assessment activity meets organisational, safeguarding, and regulatory requirements. Maintain confidentiality and handle sensitive information appropriately at all times.
Essential Knowledge & Experience:
- Strong knowledge of Mental Health services (essential).
- Working knowledge of Learning Disabilities and Autism.
- Clear understanding of referral and assessment processes within health and social care.
- Experience working with managers, clinicians, or external services.
- Experience in diary management and KPI-driven environments.
Skills & Competencies:
- Excellent communication and relationship-building skills.
