Personal Advisor – Future Directions Leaving Care Team (Barnsley)

Role Overview
Join a supportive and stable team dedicated to helping care-experienced young people (ages 16–25) transition into adulthood. You’ll provide guidance across key life areas including housing, education, employment, health, and relationships.
Key Responsibilities
- Build trusted relationships with care leavers and post-care individuals
- Support and update Pathway Plans
- Advocate across multi-agency settings (housing, education, health)
- Promote access to training, employment, and education
- Provide guidance on tenancy, homelessness, and family reunification
- Address challenges around substance misuse, mental health, and disabilities
- Maintain accurate records and produce high-quality reports

Ideal Candidate Profile
- Experience with care leavers / looked-after children
- Comfortable working with 18–25 year olds (primary focus)

- Willingness to learn and seek support when needed
