Join the Community Mental Health Team for Older Adults, a collaborative and supportive environment dedicated to improving mental health services. This role focuses on conducting assessments and reviews under the Care Act 2014, ensuring individuals receive the right support to maintain independence and wellbeing.

Location: Blackpool

Contract: Temporary – 6 weeks

Hours: 37 per week

Pay: £16.62 per hour

Start Date: ASAP

Key Responsibilities

  • Complete needs-led assessments and reviews for older adults with mental health needs.
  • Develop and monitor outcome-based care plans.
  • Work closely with health professionals and external agencies for integrated care delivery.
  • Undertake capacity assessments and arrange advocacy where required.
  • Maintain accurate records and comply with statutory requirements.
  • Identify and report safeguarding concerns promptly.

Requirements

  • Enhanced Adult Workforce DBS (with Barred List check).
  • Two years of references and verified employment history (including gaps).
  • Relevant qualifications (GCSEs/NVQ2/Level 2 Diploma or equivalent in Maths and English).
  • Knowledge of Care Act 2014, Mental Capacity Act 2005, and social care policy.
  • Strong communication, IT, and record-keeping skills.
  • Experience in social care settings and inter-agency working.
