Join the REaCH team at Stockport Council, where we focus on delivering person-centred care to help individuals achieve independence. This role is crucial in supporting the physical, emotional, and mental wellbeing of service users in their homes under the guidance of registered professionals.

Day-to-day of the role:

Care Delivery

  • Deliver reablement-focused care and support based on personalised wellbeing plans.
  • Assist with personal care, medication administration, therapy tasks, and palliative care.
  • Support individuals with complex needs including dementia, mental health, substance misuse, and end-of-life care.
  • Promote independence through rehabilitation and reablement strategies.

Communication & Collaboration

  • Communicate effectively with service users, carers, and multidisciplinary teams.
  • Maintain accurate written and electronic records.
  • Escalate concerns and contribute to safeguarding procedures.

Clinical & Therapy Support

  • Undertake physiological measurements and basic clinical tasks when trained and competent.
  • Administer prescribed medication and support self-administration.
  • Participate in health promotion and wellbeing activities.

Planning & Organisation

  • Manage own workload and work independently in service users’ homes.
  • Participate in supervision, training, and service development.
  • Ensure safe use of equipment and resources.

Teamwork & Leadership

  • Support new staff through induction and shadowing.
  • Contribute to service audits and inspections.

Required Skills & Qualifications:

  • Experience in personal care and working with older adults in their homes.
  • Ability to work independently and within a team.
  • Strong written and verbal communication skills.
  • Commitment to training and development.
  • Understanding of anti-oppressive practice and safeguarding.
  • Willingness to undertake the Care Certificate.
  • Ability to drive and access to a vehicle (reasonable adjustments for disabled applicants).
  • NVQ Level 3 in Care or equivalent is desirable.
  • Medication training
