Job Description
Job Title: Advanced Clinical Practitioner – Memory Service
Location : Grimsby
Job Summary:We are seeking a dynamic and experienced Advanced Clinical Practitioner (ACP) to join our evolving Memory Service. This is a highly autonomous role requiring advanced clinical skills to assess, diagnose, and manage both organic and functional mental health conditions across adult and older adult populations. The role is integral to service transformation and demands flexibility, innovation, and a commitment to high-quality, person-centred care.
Key Responsibilities:
- Conduct comprehensive assessments, formulate diagnoses, and implement evidence-based treatment plans for a wide range of mental health conditions including dementia, psychosis, personality disorders, and co-occurring substance use.
- Work autonomously and collaboratively within multi-disciplinary teams to manage complex, high-risk cases.
- Lead and chair multi-disciplinary meetings, contributing to clinical decision-making and care planning.
- Prescribe medications within scope of practice and monitor treatment efficacy and safety.
- Provide clinical supervision, mentorship, and training to junior staff and students.
- Participate in service development, quality improvement initiatives, and implementation of NICE guidance.
- Engage in safeguarding processes and apply relevant legislation including the Mental Health Act and Mental Capacity Act.
- Contribute to audits, serious incident reviews, and structured judgement reviews to enhance clinical governance.
Essential Qualifications & Experience:
- Registered healthcare professional (NMC/HCPC or equivalent).
- MSc in Advanced Clinical Practice or equivalent.
- Non-Medical Prescribing qualification.
- Minimum 5 years post-qualification experience.
- Demonstrated competence in managing complex mental and physical health conditions.
- Experience in safeguarding, risk assessment, and crisis planning.
- Proven leadership in clinical settings and involvement in service development.
- Valid UK driving licence.
Desirable Qualifications & Experience:
- Senior-level practice development or post-graduate training in one or more ACP pillars.
- Experience in managing long-term conditions, frequent attenders, or working within PCNs.
- Leadership in serious incident investigations and implementation of learning outcomes.
- Involvement in quality improvement, change management, and audit implementation aligned with NHS priorities.
Knowledge & Skills:
- Strong leadership and change management capabilities.
- Excellent communication and interpersonal skills with service users, carers, and professionals.
- Ability to manage a complex caseload and support others in risk management.
- Deep understanding of cultural competence and anti-discriminatory practice.
- Skilled in chairing complex case discussions and influencing multi-disciplinary care planning.
Desirable Knowledge & Skills:
- Experience in all-age mental and physical health care.
- Previous management or supervisory responsibilities.
- Demonstrated impact in service innovation and sustainability.
