Our Client, Wrexham Council, is seeking an Adults Social Worker for their Adults Single Point of Access SPOA, which is a joint team with Early Response.

Responsibilities of this role:
- Screening and responding to all referrals made to the service, determining next steps, signposting and coordinating the offer of Information, Advice and Assistance, where possible.
- The team work in a collaborative and person centred way and require an experienced social worker who embodies the same values and principles.
- Conduct thorough assessments to understand a person's needs and challenges.
- Refer clients to appropriate support services, including specialist teams within the local authority, community organizations, and other health and social care professionals
Requirements of this role:
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
- Must have minimum of 3 years post qualifying experience working as a Qualified social worker
