Adult\'s Social Worker - Mental Health Multi-Disciplinary

Liverpool City Council is looking for agency social workers for its Adult Mental Health multi-disciplinary team!

Fantastic pay rate of £46.08 per hour!!

Details & Responsbilities of the role:

• Attend ward reviews to work as part of mental health multi-disciplinary teams in adults and older persons wards
• Complete assessments of need under the Care Act and develop strengths based support plans.
• Key in developing discharge plans and lead on these
• Complete Mental Capacity Assessments and make Best Interest decisions when required
• Partnership working with health, housing, children’s services etc essential

Requirements of the role:

• Qualified Social Worker
• Fully Social Work England registered
• Must have significant post-qualified experience in adult social care
• Must have Mental Health experience
