Responsibilities of this role:
- Conducting thorough assessments to identify individual needs, including social, psychological, and practical requirements, to develop personalized care plans.
- Collaborating with hospital staff, community services, and other relevant professionals to plan and coordinate a patient's safe and effective discharge from the hospital.
- Creating and implementing care plans that address the individual's needs, promote independence, and facilitate recovery, often involving reablement and rehabilitation goals.
- Connecting patients with appropriate community services, such as home care, therapy, support groups, and financial assistance, to ensure ongoing support.
- Providing emotional support, advocacy, and guidance to patients and their families throughout the intermediate care process.
- Identifying and addressing any safeguarding concerns related to vulnerable individuals, ensuring their safety and well-being.
- Working effectively with a range of healthcare professionals, including nurses, therapists, doctors, and other social workers, to ensure a holistic approach to patient care.
Requirements of this role:
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have a Social Work qualification (Diploma in social work, social work degree or equivalent).
- Must have minimum of 3 years experience post qualifying working as a Qualified social worker.
