  • Working with adults with Learning Disabilities and Autism

  • You will undertake assessments in line with the Care Act

  • You will commission services in line with individuals’ budgets

  • You need to be able to carry out complex assessments and detailed evidence-based good quality reports, letters etc. including court reports

  • You will carry out safeguarding enquiries under Section 42, investigating and managing potential risk

  • You may be involved in Court of Protection work

  • Mental capacity assessments

  • Deprivation of liberty work

  • Participating in CPA meetings in partnership with health professionals, for services users with complex care needs and at risk of being sectioned under Section 2 of the Mental Health Act.

  • Undertaking outcomes focused reviews

  • Completing Continuing Health Checklist (CHC), participating in Decision Support Tool (DST) Meetings and working alongside to meet citizens’ health and social care needs.

  • Using TOM (Target Operating Model).

  • Office base is County Hall (LE3 8RA) although your work will cover the Hinckley area

  • You are required to be office based once per week on duty

  • Qualified Social Worker

  • Fully Social Work England registered

  • Must have significant experience in adult social care

  • Ideal candidate will have specialist experience working with adults with learning disabilities
