About Us:

We are looking for a Personalised Support Plan Consultant who is passionate about tailoring care solutions to meet the unique needs of individuals, ensuring they live independently and thrive in their own homes.
Role Overview:
As a Personalised Support Plan Consultant, you will work directly with clients to create bespoke care plans that meet their physical, emotional, and technological needs. You will collaborate with a multidisciplinary team to ensure the seamless delivery of care services.
Key Responsibilities:
- Conduct assessments and develop personalised support plans based on individual needs.
- Collaborate with care teams to ensure that support plans are implemented effectively.
- Regularly review and adjust care plans to reflect changes in clients’ circumstances.
- Provide training and guidance to clients and their families on how to maximise the support plans.
- Ensure compliance with relevant standards and regulations.
Qualifications & Skills:
- Experience in creating personalised care plans, ideally in a social care setting.
- Strong understanding of care management and person-centred approaches.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
