As part of this fast-paced, high-impact team, you’ll be working at the frontline of integrated care, where your decisions directly influence outcomes for individuals and their families. You’ll collaborate closely with NHS colleagues, discharge coordinators, therapists, and our voluntary and community sector to ensure that people are supported holistically—not just to leave hospital, but to thrive once they do.
We are seeking to recruit 1 qualified Social Worker with strong ethics and values to support adults and carers in our communities to live the life they choose to, regardless of their age or stage of life.
You’ll be applying the Care Act 2014 to assess needs, manage risk, and coordinate support that promotes independence, dignity, and wellbeing. Whether it’s a short-term reablement package, community-based support, or safeguarding intervention, you’ll be central to delivering the right care, in the right place, at the right time.

What You’ll Do
- Lead social care assessments for hospital patients in line with the Care Act 2014
- Facilitate safe, timely discharges that prioritise recovery at home wherever possible
- Work collaboratively with health professionals, therapists, and voluntary sector partners
- Identify and manage safeguarding concerns and complex needs
- Support carers and families through discharge planning and transitions
- Promote independence, choice, and person-centred care
What You’ll Bring
- Social Work England registration
- Experience in hospital discharge, integrated care, or adult social work
- Strong understanding of the Care Act, Mental Capacity Act, and discharge pathways
- Excellent communication, assessment, and coordination skills
- Ability to work under pressure and manage competing priorities
- A commitment to strengths-based, preventative practice
