Join our Sensory Team to empower Deaf, Hard of Hearing, and dually sensory-impaired adults in Hammersmith & Fulham. Conduct specialist assessments, craft person-centred support plans, and promote independence under the Care Act 2014.

Key Responsibilities
  • Carry out initial assessments & reviews for adults with sight/hearing loss.
  • Design strength-based care plans, including direct payment options.
  • Provide/demonstrate assistive equipment & collaborate with audiology teams.
  • Advocate for service users & raise sensory impairment awareness borough-wide.
  • Maintain Mosaic records & liaise with multi-agency partners.
What We’re Looking For
  • BSL Level 2 (essential).
  • Care Act 2014 & strengths-based practice knowledge.
  • NVQ Level 3/A-Level equivalent; social care/sensory impairment experience.
  • Excellent communication, IT skills (Mosaic training provided), & problem-solving.
  • Self-motivated, resilient, with advocacy experience.
