- Overview
A compassionate and proactive Hope Worker will support individuals affected by substance misuse, promoting recovery, reducing harm, and encouraging long‑term positive change.
You will work closely with service users, healthcare professionals, pharmacies, and partner agencies across community venues, outreach locations, and service users' homes.
Key Responsibilities
- Support individuals to achieve and sustain positive recovery outcomes while reducing risks associated with substance misuse.
- Complete assessments, risk assessments, support plans, and regular reviews.
- Deliver brief and extended interventions that encourage behavioural change.
- Undertake walk‑in assessments, referrals, and triage activities.
- Collaborate with Recovery Coordinators to support structured recovery pathways.
- Provide practical support to improve wellbeing, resilience, confidence, and community engagement.
- Deliver outreach, in‑reach, and cross‑reach activities; encourage engagement with treatment and recovery plans.
- Work collaboratively with families, carers, community organisations, and partner agencies to support access to healthcare, housing, welfare benefits, education, training, and employment.
- Provide advice and education on safer drug use, safer sex, and blood‑borne viruses (including HIV and Hepatitis B/C).
- Support needle‑exchange services in fixed‑site and community settings and collaborate with pharmacies to increase access to naloxone.
- Deliver harm‑reduction training and contribute to health promotion and prevention initiatives.
- Support adults with complex needs, including substance misuse, homelessness, mental health challenges, offending behaviour, and social isolation.
- Deliver training and awareness sessions to colleagues, partner organisations, and the wider community.
- Maintain accurate case records, reports, and documentation.
- Participate in supervision, training, and continuous professional development.
Qualifications
- NVQ Level 3 in Health & Social Care (or equivalent) or willingness to work towards achieving it.
- Experience supporting vulnerable adults affected by substance misuse, mental health challenges, homelessness, or social exclusion.
- Strong communication, organisational, and interpersonal skills.
- Ability to build effective professional relationships while maintaining appropriate boundaries.
- Confidence working independently in community settings and collaboratively as part of a multidisciplinary team.
- Flexible, proactive approach with the ability to respond to varying levels of need.
- Desirable: Experience working in outreach or community‑based services.
- Desirable: Knowledge of harm‑reduction and recovery‑oriented practice.
