The predicted salary is between 22100 - 28900 £ per year.

Location: 74 Kirkgate, Leeds, LS2 7DJ

Working Hours: Monday-Friday 8am-4pm or 8:30am-4:30pm. This post will include some early morning and evening work (Monday-Friday) on a rota basis (the team currently do a late shift 10am-6pm and an early shift 5:30am-12:30pm, both occur approximately once every 6 weeks).

Contract Type: Permanent

Salary: £25,110 - £32,090 per annum

About the Role

An exciting opportunity has become available for the position of Street Outreach Practitioner based within our Forward Leeds team.

What You’ll Do

  • Manage a caseload, devising, implementing, and reviewing ongoing recovery and treatment plans.
  • Facilitate and promote individuals’ progression from initial engagement into structured treatment.
  • Work in a proactive role to minimise and prevent rough sleeping and begging within Leeds by engaging and maintaining clients into treatment.
  • Reduce substance related harm to individuals and the wider community.
  • Work collaboratively with key partners and colleagues across the street support partnership to achieve the best health and wellbeing outcomes for individuals.
  • Support individuals to move into supported, more permanent accommodation.
  • Provide holistic support to adult service users and coordinate their drug treatment, including assessment, recovery coordination, Psychosocial Interventions (PSI) delivery and multi-agency working.
  • As directed by the Lead Practitioner, be an integral member of the dynamic multi-agency team developing comprehensive and tailored individual interventions.

To Succeed in This Role, You’ll Need:

  • Experience of holding a caseload.
  • Knowledge and/or experience of substance misuse.
  • Experience supporting vulnerable people, whose lives have been affected by alcohol and/or drug use.
  • A relevant qualification in Health and Social Care.
