About The Role
Practitioners provide advice, assessment, brief and structured interventions, group programmes and training courses. Practitioners are expected to work not only from Aquarius offices but also in primary care, arrest referral, hospital and youth settings and other outreach settings as required by the service specification.
Duties and Responsibilities
- Working in a way that recognises and values diversity and with an understanding of the functional model of substance misuse, the model of change, CBT and a commitment to Recovery, the practitioner will:
- Provide advice, screening, brief and comprehensive assessments with individuals, families or couples.
- Provide brief and longer-term structured interventions.
- Develop a care plan and ensure it is implemented and reviewed with the client(s) and monitor outcomes.
- Provide case management and ensure integrated pathways.
- Provide advice and structured support for affected others.
- Provide group programmes and support groups.
- Maintain written records and data.
