Aquarius offers the perfect environment to put your skills to great use as a Complex Needs Practitioner working within the Rough Sleepers Initiative.

Aquarius is a charity dedicated to helping people and communities overcome the harm caused by alcohol, drugs, and gambling. We are part of β€˜One Recovery Buckinghamshire’ (ORB), a partnership between Inclusion (MPFT NHS Trust), Aquarius, and Build on Belief, providing integrated support across the county.

We are recruiting a Complex Needs Practitioner to support individuals experiencing rough sleeping and complex needs. Your focus will be on engaging people through a person-centred approach, empowering them to reduce risks, increase resilience, and achieve positive outcomes in housing stability, health, and wellbeing.

Working flexibly across the community, multi-agency partnership settings, and outreach environments, you will manage a caseload including referrals from housing, social care, and health sectors. You will join a caring, multi-disciplinary team delivering tailored psychosocial interventions to support diverse client groups.

Key Responsibilities:

  • Engage rough sleepers and individuals at risk of homelessness, providing holistic support addressing substance use, physical and mental health, relationships, and community engagement.
  • Work proactively to assist clients in securing stable accommodation and accessing relevant health and social care services.
  • Collaborate closely with statutory and voluntary agencies, housing providers, and healthcare professionals to deliver coordinated support.
  • Maintain accurate records and reports reflecting client progress and outcomes.
  • Provide outreach support, working flexibly across community locations with a central hub based at our head office.

Person Specification:

  • Qualification in health/social care, youth/community work (e.g., NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies) OR relevant experience working with rough sleepers or people with complex needs, with commitment to NVQ Level 3.
  • Proven experience engaging individuals with complex needs, particularly rough sleepers or people affected by substance misuse.
  • Ability to work independently, showing energy, confidence, and flexibility.
  • Excellent communication, record keeping, and report writing skills.
  • A commitment to multi-agency working and outreach support.
