Location: Likewise Portland House 4 days per week and 1 day per week at Wicker
Working Hours: 37 hours, Monday - Friday with an 11am - 7pm clinic once per fortnight

Contract: Permanent
Salary: £40,266 - £54,478

As a Health and Wellbeing Nurse, you will work as an integral member of the wider clinical team within the Likewise Sheffield service, providing high-quality, person-centred nursing care to people experiencing drug and/or alcohol-related difficulties. You will promote physical and psychological wellbeing, reduce health inequalities, minimise drug- and alcohol-related harm, and support service users to access appropriate healthcare and treatment.

What You’ll Do

• Carry out alcohol assessments and support community alcohol detoxifications, including monitoring, clinical review and health education.




• Make appropriate referrals to external providers, specialist services, hospitals, GPs and other support networks, including mutual aid.




• Provide comprehensive healthcare assessments for people using drugs and/or alcohol, identifying physical health, mental health and psychosocial needs and making appropriate onward referrals.




• Undertake clinical observations, physical health monitoring and investigations, including blood tests, ECGs and other health screening, in accordance with competency, local policy and clinical need.




• Undertake blood-borne virus (BBV) screening, including hepatitis B, hepatitis C and HIV testing where appropriate, provide results in a sensitive and supportive manner, and facilitate appropriate onward referral and treatment.




• Provide and coordinate hepatitis A and B vaccinations, following local pathways, national guidance and service procedures.




• Promote harm reduction, including safer injecting advice, overdose prevention, wound care and appropriate provision of naloxone.




• Identify and respond to drug and alcohol related physical health complications, including injection-related injuries, infections, poor nutrition, liver disease and other long-term health conditions.




• Deliver sexual health interventions, including testing, education, and onward referral to specialist sexual health services where required.




• Provide health promotion and prevention interventions relating to smoking, including nicotine replacement therapy and referral to appropriate smoking cessation services.




• Support service users to access routine and preventative healthcare, including GP services, screening programmes, dental services and other relevant healthcare providers.




• Support people who may experience significant barriers to healthcare as a result of stigma, health inequalities, homelessness, social exclusion or complex needs.




• Work collaboratively with GPs, community pharmacies, hospitals, sexual health services, hepatology services, mental health services and other partner agencies to ensure effective continuity of care.




• Recognise and respond appropriately to safeguarding concerns, deteriorating physical or mental health, and other clinical risks, escalating concerns in accordance with local procedures.




• Maintain accurate, contemporaneous and confidential clinical records and ensure all care, interventions, referrals and outcomes are appropriately documented.




• Promote a trauma-informed, compassionate, non-judgemental and person-centred approach, recognising the impact of stigma and previous experiences of healthcare on engagement.




• Work within professional standards, scope of practice, local policies, clinical guidelines and relevant legislation, maintaining professional competence and continuing professional development.

To Succeed in This Role, You’ll Need

• A strong commitment to working with people experiencing drug and/or alcohol-related difficulties and a genuine interest in promoting health equality.




• Registration as a Registered Mental Health Nurse (RMN) or Registered General/Adult Nurse (RGN), with the qualifications, knowledge and professional commitment required to provide safe and effective clinical care.




• Strong clinical assessment and decision‑making skills, with the ability to identify and respond to physical, psychological and social needs.




• Clinical knowledge of blood‑borne viruses, including hepatitis C, hepatitis B
