We are seeking a compassionate, skilled carer support worker to work wake nights as part of the existing care team. Our client is a 19 year old female with complex needs and cerebral palsy. You will work closely with the team, case manager and wider multi-disciplinary team to ensure person-centred care is delivered whilst following the best practice legislation. Experience in PEG feeding and tracheostomy & stoma care is essential.
Duties:
- Assist clients with daily activities including personal hygiene, dressing, and mobility support
- Administer medication in accordance with prescribed care plans and medical guidelines
- Support clients with PEG feed preparation and nutritional needs, ensuring dietary requirements are met
- Maintain accurate documentation of care provided and any changes in client condition
- Support client with medical appointments and accompany them when necessary, including driving to appointments
- Assist with light housekeeping tasks to maintain a clean and safe environment
- Collaborate with healthcare professionals to develop and review personalised care plans
- Respond promptly to emergencies or urgent needs following first aid protocols
Skills:
- Proven experience in complex care essential.
- Knowledge of medical terminology, medication administration, and case management principles
- Excellent communication skills with the ability to engage effectively with family and the MDT
- Ability to use safe manual handling techniques, when required and assist with mobility support
- Valid driving licence for transporting client safely to appointments or activities essential.
- First aid certification is preferred but not essential; training can be provided for suitable candidates
- Compassionate nature with patience and empathy for vulnerable individuals requiring specialised support
