We are currently recruiting for an experienced Band 7 Children's CHC Nurse Assessor to support an NHS Integrated Care Board in London. This is an excellent opportunity for an experienced Continuing Care professional to join a busy team responsible for assessing, reviewing and coordinating care packages for children and young people with complex health needs.
Key Responsibilities:
- Undertake Children's Continuing Care assessments and reviews in line with the National Framework and local policies.
- Assess the healthcare needs of children and young people with complex needs.
- Produce high quality assessment reports and recommendations.
- Participate in multidisciplinary team meetings and case discussions.
- Work collaboratively with families, healthcare professionals, education providers and local authority colleagues.
- Support care planning and ongoing review processes.
- Ensure accurate and timely documentation is maintained.
Essential Requirements:
- Registered Nurse with current NMC registration.
- Previous experience undertaking Children's Continuing Care (CHC) assessments.
- Strong understanding of Continuing Care processes and frameworks.
- Experience working within NHS services.
- Excellent communication, report writing and stakeholder management skills.
- Ability to work autonomously and manage a varied caseload.
