Overview

We are looking for a qualified Social Worker to join the Learning Disability and Neurodiversity Team within the Adult Social Care team at Reading Borough Council. We’re committed to offering the support and development you need.

Responsibilities

  • Complete timely Care Act assessments for higher intensity case management with a strength-based focus, delivering positive outcomes for individuals in Reading.
  • Complete post-discharge reviews to ensure care plans are accurate and individuals’ needs are being met.
  • Promote the use of direct payments to give individuals flexibility in how they meet their needs.
  • Promote a reablement focus for all individuals referred to adult social care.
  • Work with families and carers to ensure carers assessments are completed to support individuals with a caring role.
  • Develop close and effective relationships with providers, community voluntary services, and health professionals.
  • Complete Mental Capacity Act Assessments to a high standard.
  • Undertake community Deprivation of Liberty Safeguards (DOLs) and prepare records for the Court of Protection.
  • Be part of a high performing, efficient, supportive and outcome-focused team embedding the principles of TEAM Reading.

Team Objectives

The Learning Disability and Neurodiversity (LDN) Team works closely with other teams in Adult Services involved in delivering Adult Social Care. We work with individuals from transitions to adulthood and older age, promoting a strength-based ethos to achieve the best outcomes. We aim to be a high performing, multidisciplinary team with a clear purpose and values that support staff and celebrate diversity.

What You’ll Do

  • Hold a varied caseload and contribute to team performance and positive outcomes for residents.
  • Access high-quality, client-led interventions in residents’ own homes within a friendly and supportive team.
  • Work within the legal framework provided by statute, guidance, policies and procedures.

Qualifications & Requirements

  • A degree or equivalent in Social Work
  • Registration with Social Work England
  • Excellent understanding of the Care Act 2014
  • Understanding of The Autism Act 2009
  • Understanding of Better care for people with learning disabilities and people with autism
  • Other legislation linked to Neurodiversity and Learning Disability
