We are seeking a dedicated and compassionate Social Care Coordinator to join our dynamic Review Team within Adult Social Care. This is an exciting opportunity to make a meaningful difference in the lives of adults in need of care and support.

About the role
The Review Team is a friendly and supportive multidisciplinary team undertaking annual Care Act reviews with adults receiving support services. Social Care Coordinators work with individuals to undertake assessments, reviews and care and support planning in line with Care Act 2014, Mental Capacity Act 2005, and local policies and procedures.
You will collaborate with service users, families, and multidisciplinary teams to develop and maintain person-centred care plans. Key to this is a strength-based approach, supporting people to use preventative services, and to build resilience and achieve outcomes that are important to them, involving the person and supporting them to have choice and control. It is also important to maintain accurate records and ensure all documentation is up to date.
The role offers an apprenticeship element which entails study time provided as part of your working week. The role also involves supporting first line queries into the team and working closely with other departments within Reading Borough Council to resolve these. Data input and management is a key part of the role, which is identified in elements of the apprenticeship.

Experience of working with people with care and support needs is preferred, however transferable skills will be considered, and we will provide the right training for the right candidate. The apprenticeship will support your growth in the role.
