We are currently seeking a dynamic Social Care Coordinator to join our Advice and Wellbeing Hub – Social Work Team. We represent a Local Authority dedicated to enhancing the well-being of our community and residents. We are seeking an individual with excellent communication abilities, adept at building and fostering relationships with colleagues, service users, healthcare providers, and community networks.

In the role of Social Care Coordinator at Reading Borough Council, you will play a crucial role within a friendly and cooperative team. You will collaborate with other Social Care Coordinators, Occupational Therapists, and professionals from the voluntary and community sector. The Advice and Wellbeing Hub (The Hub) aims to actively promote wellbeing and independence, supporting people to retain or regain their skills and confidence by using existing and new support networks, Technology Enabled Care (TEC), equipment and short-term interventions.

About the Role: In the role of a Social Care Coordinator, your responsibilities will involve making home visits to individuals. You will undertake non-complex assessments of people’s needs and develop and monitor plans in collaboration with service users themselves. The objective is to enhance their independence through a strengths-based methodology and a person-centred approach. You will hold a varied caseload and will be expected to work independently and to interpret and analyse varied information or situations and to produce solutions. You might evaluate and suggest basic equipment or TEC, support goal planning, offer short-term care or enablement, and work closely with care providers, Housing Services, healthcare peers, and the voluntary and community sector. You will also support the team’s Duty function triaging cases that come into the team with support from Social Workers and other members of the team. You will work within the legal framework provided by the statute, guidance, policies, and procedures.

Experience working with people and experience working within the Health, Adult Social Care or voluntary sector is required.
