Join us as a Community Assessor and make a real difference in people’s everyday lives. You’ll work closely with residents, families and professionals to assess needs, explore options for support, and connect individuals with local resources that help them stay safe, well and independent. This is a varied and rewarding role where no two days are the same - one day you could be carrying out home visits, and the next you might be helping someone access the services they need to regain their confidence and independence.

About The Role: You’ll carry out person-centred assessments in people’s homes and in the community, working closely with social workers, occupational therapists, and health professionals to develop effective care and support plans. Your work will make a real difference in improving outcomes and promoting independence for adults and their carers. You will be responsible for writing detailed care and support plans for individuals with eligible needs, ensuring your assessments are clearly evidenced. As part of this process, you will make appropriate referrals for financial assessments and complete comprehensive risk assessments to support safe and effective care planning. This role follows a 7-day fortnight rota, meaning you’ll work:

  • Week 1: Saturday, Sunday, Wednesday, Thursday
  • Week 2: Monday, Tuesday, and Friday

Key Responsibilities:

  • Undertake holistic assessments of adults and carers in line with the Care Act 2014.
  • Identify support needs and work collaboratively to develop appropriate care plans.
  • Provide advice, guidance and signposting to community resources and preventative services.
  • Review existing care and support packages to ensure they remain effective and person-centred.
  • Maintain accurate records and contribute to the continuous improvement of services.

About You: You’ll have experience of working with vulnerable adults or within a health or social care setting, with a good understanding of community support networks and the principles of person-centred care. Strong communication skills, empathy and the ability to work independently are essential. You’ll also need to be confident using IT systems and have excellent organisational skills to manage your caseload effectively. You’ll be part of a team that relies on each individual to play their part to the best of their ability.
