This role provides a safe and cost‑effective medicines management pharmacy service to patients cared for by a designated Urgent Community Response (UCR) team within the Eastern Community Services Division, based in the Exeter Locality.

Responsibilities

  • To provide a safe and cost‑effective medicines management pharmacy service to patients who are cared for by a designated Urgent Community Response (UCR) team within the Eastern Community Services Division.
  • To clinically assess and review the medication‑related needs of patients on the Urgent Community Response caseload.
  • To be a source of medicines advice to colleagues and key stakeholders with respect to medicines optimisation for patients to support integration across health and social care.
  • To promote self‑administration of medication through focusing on adherence interventions for individual patients.

Working Pattern

Monday – Friday 8am – 5pm
