An exciting opportunity has arisen within the renal community team. The role spans pre-dialysis clinics and education to managing renal patients at home on their chosen therapy. The renal unit covers a large geographical area in Devon and Somerset.
About the Role
The main duties of this role are to:
- Assess, develop and implement specialist renal nursing care programmes and provide advice, education, support and care to people with chronic / end stage kidney disease in the community setting.
- Provide specialist advice to primary and secondary care teams in geographical caseload area.
- Provide ongoing emotional and psychological support to patients and their families.
- Participate in and evaluate research and clinical audit in relation to specialty to inform and improve future practice.
Working Pattern: 37.5 hours per week. Monday - Friday 0800-1600hr
