Are you seeking a new challenge working in a patient facing role in Community Services? We have a position available for an experienced pharmacist to join our established Community Services Pharmacy Team at Exmouth Community Hospital. The post holder will enable the safe and effective use of medicines through provision of medicines optimisation and personalised medication reviews as part of a wider multi disciplinary team.

Responsibilities

  • Providing medicines advice to patients, carers, health and social care professionals and skilled non-registered staff working collaboratively to resolve complex medication queries.
  • Medicines reconciliation on admission and discharge and ensuring clear onward communication to our primary care colleagues.
  • Undertaking clinical medication reviews and making evidence-based clinical recommendations to ensure high quality, patient-centred care.
  • Empowering patients to manage their medication safely, with support for developing confidence and capability in self‑administration.
  • Multidisciplinary collaboration with medical, advanced clinical practitioners, nurses, therapists, primary care clinicians, community pharmacy, mental health teams, care providers and social care staff professionals.
  • Providing a high quality, safe and patient focused clinical pharmacist service to the Community Care Group, including community hospital inpatient units, outpatient clinics, ambulatory clinics and patients’ home settings to meet professional, Care Group and Trust requirements.
  • Delivering high quality personalised patient care in line with legislation, national and local guidance and specifications.
  • Optimising clinical outcomes, improving medicines safety and delivering best value from medicines within community services.
  • Working towards a non-medical prescribing qualification, qualifying and working as an independent prescriber within a defined patient caseload.
  • Participating in the wider pharmacy service and its development in response to patient care needs.
  • Contributing to the development of medicines management and optimisation throughout the patient care pathway.
  • Providing supervision, training and mentoring of staff, including postgraduate and undergraduate pharmacist placements.
  • Supporting the wider community services pharmacy team by delivering training to other members of the multidisciplinary team on prescribing and medicines related topics.

Working Pattern

22.5 hours per week.
