More detail about the role

This is an exciting opportunity to combine advanced clinical practice with clinical leadership, managing a dedicated team of Heart Failure Nurse Specialists while continuing to provide expert care for patients across both acute and community settings.

As one of the Clinical Heart Failure Leads, you will play a pivotal role in delivering our integrated Heart Failure service, including our NICE-compliant one‑stop suspected Heart failure diagnostic clinics, nurse‑led follow‑up services, inpatient reviews, community care and medication optimisation. You will work closely with consultant cardiologists, the wider multidisciplinary team and primary care colleagues to deliver high quality, patient centred care and drive service development.

We are looking for an experienced Heart Failure practitioner with excellent clinical assessment skills, an independent non‑medical prescribing qualification, proven leadership ability and a passion for service improvement. You will have experience of managing complex patients, supporting service development and contributing to quality improvement through audit and evidence based practice.

In return, we offer the opportunity to work within a supportive cardiology team, contribute to the future development of Heart Failure services across North Devon and the wider Trust and access excellent professional development opportunities.

If you are ready to help lead a specialist service and make a real difference to the lives of patients living with Heart Failure, we would love to hear from you.
