Are you looking for an exciting new opportunity? Camden Health Improvement Practice is a multidisciplinary service providing health care to homeless people (street homeless and temporarily housed individuals) and those with substance misuse, alcohol and mental health problems, within the Borough of Camden.
Main Responsibilities:
- Providing care for the presenting patient, treatment, chronic disease management and evaluation of care.
- Providing health education and health promotion advice and information to clients on health related issues such as harm minimisation (drugs, alcohol, smoking cessation etc), women’s health, mental health, safer sex etc.
- Working as an autonomous practitioner within a clinical team of GPs, Nurses and Physician Associates, providing evidence based nursing care to meet the health needs of this vulnerable client group.
- Assisting the registered manager with duties related to CQC outcomes, ensuring that compliance is maintained.
- Working collaboratively with the general practice team to meet the needs of the patients, ensuring practice policies and procedures are followed.
Ideal Candidate:
- UK registered nurse with significant post registration experience.
- Experience in cervical cytology.
- Must have a good knowledge of the quality outcomes framework.
Package Details:
- Working hours: Monday to Friday 9am - 5pm, 27.5 hours per week.
