Job Brief

The Care Coordinator provides high-quality, person-centred care while taking additional responsibility for the safe administration, recording and monitoring of medication.

The role combines the practical duties of a Healthcare Assistant with medication support and day-to-day coordination of care.

The postholder will support service users with personal care, daily living and wellbeing while helping ensure that care plans, medication records and staff handovers are accurate and up to date.

Responsibilities

Key responsibilities Care and support

  • Provide respectful, person-centred care in accordance with each service user’s care plan.
  • Assist with personal care, dressing, mobility, continence, nutrition and hydration.
  • Encourage independence, dignity, choice and social participation.
  • Observe service users’ physical and emotional wellbeing and report any concerns promptly.
  • Maintain professional boundaries and protect service users from abuse, neglect and avoidable harm.
  • Respond appropriately to emergencies, incidents and changes in a service user’s condition.

Medication responsibilities

  • Administer or support service users with medication only after completing the required training and competency assessments.
  • Follow medication administration records (MAR charts), care plans, prescriptions and organisational policies.
  • Complete medication records accurately and immediately after administration.
  • Follow the correct procedures for refused, missed, delayed or unavailable medication.
  • Monitor and report possible side effects, adverse reactions, errors or changes in a service user’s condition.
  • Carry out medication stock checks and support the ordering, receipt, storage and disposal of medicines.
  • Maintain secure storage of medication and follow requirements for controlled drugs where applicable.
  • Escalate medication concerns promptly to the manager, pharmacist, prescriber or relevant healthcare professional.
  • Never administer medication outside the role’s training, assessed competence or authorised responsibilities.

Care coordination

  • Help coordinate daily care visits, tasks or shift responsibilities.
  • Support effective communication between service users, relatives, care staff and healthcare professionals.
  • Maintain accurate care notes, handover records, incident reports and other required documentation.
  • Report changes that may require a review of a service user’s care plan or risk assessment.
  • Support new staff during induction and demonstrate safe working practices.
  • Assist with quality checks, medication audits and the investigation of recording discrepancies.
  • Provide clear handovers and ensure important information is passed to the appropriate person.
  • Contact staff or managers when cover, additional support or urgent action is required.

Person specification Essential

  • Previous experience as a Healthcare Assistant, Care Assistant or Support Worker.
  • Experience of providing personal care and person-centred support.
  • Medication administration training, or willingness to complete it before administering medication.
  • Ability to demonstrate and maintain medication competency.
  • Good written, verbal and numerical skills.
  • Ability to complete clear and accurate records.
  • A caring, patient and respectful approach.
  • Ability to recognise concerns and escalates them appropriately.
  • Ability to work independently and as part of a team.
  • Understanding of confidentiality, safeguarding, dignity and professional boundaries.
  • Willingness to work shifts, weekends and bank holidays where required.
  • Satisfactory right-to-work, reference and DBS checks.

Desirable

  • Level 2 or Level 3 qualification in Health and Social Care, or equivalent.
  • Experience using electronic care-planning or medication-recording systems.
  • Experience coordinating care, supporting shifts or supervising colleagues.
  • Knowledge of infection prevention, moving and handling, first aid and basic life support.

Working conditions

The role may involve shift work, lone working, travel between service users, moving and handling duties, and supporting people with a range of physical or emotional needs.

Appropriate training, supervision and equipment will be provided.

Accountability

The postholder must follow all care plans, risk assessments, medication procedures and organisational policies.

Medication duties may only be carried out when the employee has received suitable training, has been assessed as competent and has been formally authorised by the employer.
