Reporting to the Senior Brokerage Manager, the post-holder will have a key role in developing and maintaining databases to support the Brokerage function. This will require close working with a wide range of Care Providers, Voluntary Organisations and Service Users. The Brokerage Assistant will ensure that information about individual placements, support packages and related costs is recorded accurately on information systems and disseminated to the relevant providers and social work/health professionals.
Main Areas of Responsibility
- liaise with Social Workers/Care Coordinators, Brokerage Officers, Contract Officers and Providers, Finance and other teams within Adult Social Services to ensure they receive up to date information on contracts and service users and their care;
- maintain a care home vacancy database;
- monitor compliance of systems by independent care providers and report non‑compliance to the Contract Officers;
- liaise with providers regarding service provision;
- support the development of policies and procedures;
- support service users’ and carers’ involvement in all aspects of service delivery;
- participate in the Duty system operated by the team filtering and acting on telephone calls, emails and other communications;
- ensure that the Mosaic and CM2000 systems are updated as required;
- work effectively with the Information and Assessment Team, Care Management Teams, Hospital Discharge and Finance Teams;
- maintain clear lines of responsibility and accountability in carrying out the brokerage function;
- maintain accuracy in record keeping and ensure consistent recording practice on Mosaic to provide robust financial information for budgeting, forecasting and community care charging purposes.
Other Duties and Responsibilities
- demonstrate awareness of the organisational context and commitment of the Council’s organisational values and beliefs, especially Hackney Council’s Diversity & Equality and Health and Safety Policies;
- ensure corporate standards for customer service are met and that sensitive and confidential information and political matters are handled discretely and sensitively at all times;
- ensure the way you work reflects the principles of personalisation, providing service users with person‑centred support that facilitates their ability to choose services that manage risk, achieve outcomes and promote independence;
- undertake any other duties that might arise from time to time as required within the overall grading and general responsibilities of the post.
Technical Experience / Skills / Knowledge
- Experience of working within a health or social care context;
- Experience of residential or supported living placement finding;
- Experience of managing relationships with stakeholders in a health or social care context;
- Experience of making effective use of IT systems to record and report on brokerage activity, customer contact and service performance.

- able to communicate effectively, confidently and assertively in writing and speaking;
