Ravenscroft Medical Centre is in the heart of Golders Green serving a dynamic multicultural population of 6000 patients. We are looking for a Clinical Pharmacist to work 17.5 hours per week. Our clinical team consists of 5 GPs, 1 ACP, 1 Practice Nurse and other PCN clinical staff. We are looking for a Clinical Pharmacist to be part of our team who is passionate, motivated, and willing to adapt to our modern general practice ethos.

Main duties of the job

  • The post holder will work as part of a multi-disciplinary team to:
  • Optimise the use of medicines
  • Improve the safety and quality of prescribing
  • Perform specific elements of general practices clinical workload
  • The post holder will be an independent prescriber or working towards an independent prescribing qualification.

    Ravenscroft is a medium sized practice, looking after patients from multicultural backgrounds. Our practice works alongside our PCN with supporting clinical services across the southwest of Barnet. We are a close working team and need a Clinical Pharmacist to add to our diverse team who have been with the practice for more than 20 years.

    Job responsibilities

    The following are the core responsibilities of the Clinical Pharmacist. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

    • Long-term condition clinics
    • Care home medication reviews
    • Patient facing domiciliary clinical medication review
    • Patient facing medicines support
    • Medicine information to practice staff and patients
    • Provide follow up for patients to monitor the effect of any changes.
    • Unplanned hospital admissions
    • Management of medicines at discharge from hospital
    • Repeat prescribing
    • Risks stratification
    • Service development
    • Medicine quality improvement
    • Education and training
    • Care Quality Commission

    Person Specification

    Qualifications

    • Mandatory registration with General Pharmaceutical Council
    • Minimum of two years post qualification experience
    • Independent prescriber
    • Evidence of continuing professional development
    • The post holder must either have completed the CPPE primary care pathway or be working towards it
    • UK driving licence
    • UK work permit (if required)
    • Clinical diploma or other post graduate qualification

    Experience

    • Minimum of two years post-qualification experience
    • Appreciation of General Practice and working alongside General practice staff
    • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
    • In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare including clinical audit
