HeadofAgeWellJobdescription,keypurpose HeadofAgeWellThisroleisakeypartofthecommissioningteamwhichisfundedbySouthwarkCouncilandSELICS.Theroleleadsacommissioningteamfocusedonolderand/orfrailresidents,includingphysicaldisabilities,ofSouthwark.Thepostholderwillhaveasignificantroleinbuildingworkingrelationshipsandcollaborationacrossthecouncil,thewiderhealthsystemandthroughPartnershipSouthwark. . HeadofAgeWellwillalsosignificantlycontributetoplanningforcontinuousreductioninhealthandwellbeinginequalitiesacrosstheboroughincollaborationacrossthecouncil,thewiderhealthsystemandthroughPartnershipSouthwark.Workingacrossthedivision,thepostholderwilltakestockofthecurrentcommissioningarrangementsandthestatusofpublichealth-commissionedservices,engagingwithPublicHealthConsultants,HeadsofProgramme,andPolicyOfficerstodevelopandimplementacommissioningplanforthedivision. HeadofAgeWellshouldassess,rationalise,anddeliverallaspectsofhighqualitypublichealthcommissioningwithaviewtotacklinginequalities,,withintheCouncil,theNHS,voluntarysectorandotherrelevantlocalpartners,aswellascommissionedserviceproviders.Thepostholderwillberequiredtoattendoccasionaleveningandweekendmeetingsandeventsinavarietyofsettingsandlocationsacrosstheborough,insupportofthedeliveryoftheirworkprogramme.ThepostholderreportstotheAssistantDirectorforIntegratedCommissioning.
