Role Description

This is a full-time on-site role for a Care Coordinator at Ranger Home Care in Farnham. The Care Coordinator will be responsible for care coordination, case management, communication, home care, and customer service tasks on a day-to-day basis. This job role involves visiting clients across the counties of Hampshire, Surrey, London and Sussex. Case managing clients and liaising with live in carers. Writing care plans, completing risk assessments and contributing to ensuring at all time our clients have the right match of a carer. Our team are professional, fun and work orientated with a strong commitment to caring for our clients and our carers.

Qualifications

  • Care Coordination and Case Management skills
  • Strong Communication and Customer Service skills
  • Experience in Home Care industry preferable but not essential we offer full training
  • Empathy and compassion for clients and their families
  • Ability to organize and prioritize tasks effectively
  • Excellent problem-solving abilities
  • Previous experience in the care industry is a plus
  • Certification in care coordination or related field is beneficial but not essential
  • Willingness to develop in the industry and engage in the RQF programme
  • Car driver, Full UK licence essential
