Due to the increased demands across Older People's Services, Randstad are keen to recruit experienced Social Care Co-ordinators who will become an integral part of an experienced Social Care team providing invaluable support to senior Social Workers and management teams in the delivery & provision of efficient and effective services to children & families across Northern Ireland.

Requirements:
- NISCC Registration Essential
- Candidates will be subject to enhanced vetting / compliance checks including an Enhanced Access NI check due to the nature of this work, this may include a Barred List check for both Children & Vulnerable Adults (depending on the role)
- 6-12 months previous Social Care experience essential
- Previous experience with older people would be highly advantageous
- Social Work Degree, BTEC or NVQ Level 3 in a Health & Social Care discipline essential
- NVQ Level 2 with sufficient work based experience also considered
- Willingness to undergo an Occupational Health check including submission of immunisation records as required
- Valid UK Driving licence essential
- Effective communicator
- Ability to prioritise and manage a varied caseload
- Ability to build rapport with parents, young people & social care professionals
Responsibilities:
- Work under the direction and guidance of a Social Worker on a designated caseload, taking responsibility for delegated low-risk, non-complex casework focused on prevention, confidence building, and monitoring.
- Carry out a range of person-centred activities and interventions to maximise service users' potential, promote community integration, reduce isolation, and build resilience.
- Monitor the effectiveness of domiciliary care packages and other home services through home visits, discussions with service users/family carers, and liaison with voluntary providers.
- Build a strong knowledge base of local community and voluntary networks to encourage and assist service users in utilising these resources.
- Liaise effectively with key staff from commissioned services, GPs, district nurses, and hospital staff regarding service user needs; promptly report any changes in need or risk.
- Support responsible risk-taking, contribute to risk management discussions, remain alert to indicators of changing need, and contribute to adult protection plans and strategy meetings.
- Carry out necessary administrative tasks, support the team duty system, and maintain comprehensive, accurate, and up-to-date electronic and paper records in compliance with Trust operating standards.
