Due to increased demands across community-based health teams, we are seeking an experienced Outreach Worker to join an established Substance Misuse and Recovery team.
Operating within a major regional healthcare provider, you will play a vital role in identifying, engaging, and supporting vulnerable individuals within the community.
Benefits Band 5 pay scale with competitive hourly rates. (£15.88 per hour).
Full-time hours with flexible scheduling required to meet service needs.
Weekly pay and a competitive mileage allowance for community travel.
Enhanced holiday package and access to ongoing training/e-learning support. 2-3 month contract assignment with significant scope for extension.
Pathway A: Registered Social Worker, Nurse, or Occupational Therapist on a relevant professional register, plus at least 6 months of direct experience working with adults or young people facing mental health or addiction challenges.
Pathway B: NVQ Level 3 in Care, a degree in a health-related subject (or equivalent), plus at least 1 year of relevant paid experience in a mental health or addiction setting.
Pathway C: Minimum of 3 years of experience as a Band 3 Support Worker (or equivalent within the voluntary/private sector) working directly with individuals facing mental health or addiction problems.
NISCC Registration Valid UK Driving Licence and access to a car or a reliable form of transport to complete community-based duties.
Excellent communication skills and a strictly non-judgemental approach to community care.
Proven experience in inter-agency networking or multi-sectoral collaborative work.
High proficiency in IT and Microsoft Office packages.
Providing immediate response and support to service users seeking urgent help, managing referrals until stability is reached.
Operating an outreach equipment/needle exchange service in strict adherence to agency protocols.
Monitoring the health, social, housing, and employment needs of service users, establishing direct links with hostels, housing providers, and primary health teams.
Coordinating care pathways with general practitioners, the criminal justice system (assisting individuals upon release from custody), and external counselling or advocacy networks.
Involving families in the rehabilitation process where appropriate and driving wider education strategies to reduce the stigma of addiction within local communities.
