We are seeking a skilled and empathetic Social Worker to join our team in the beautiful Scottish Borders. You will work collaboratively with adults experiencing severe and long-term mental health challenges, their families, and other professionals to deliver tailored support that promotes recovery, resilience, and independence.
This role involves:

Conducting assessments to identify individual needs, strengths, and goals.

Developing and implementing person-centred care plans.

Coordinating multi-agency support, including health, housing, and community services.

Providing advocacy and guidance to ensure service users’ rights and voices are heard.

Monitoring progress and reviewing care plans regularly.

Maintaining accurate and confidential records in line with statutory requirements.

Supporting crisis intervention and safeguarding where necessary.
