As a Social Worker within the Transformation Older Person Mental Health Team, you will play a pivotal role in delivering high-quality, person-centred care to older adults experiencing mental health challenges. Your responsibilities will encompass comprehensive assessments, care planning, and collaborative interventions aimed at enhancing the well-being and independence of service users.
Essential Qualifications & Experience
- Professional Qualification: Degree in Social Work or equivalent.
- Registration: Current registration with Social Work England.
- Experience: Minimum of 2 years post-qualification experience in adult social work, with a focus on mental health and older adults.
- Legislative Knowledge: In-depth understanding of relevant legislation, including the Mental Health Act, Mental Capacity Act, and Care Act.
- Communication Skills: Excellent verbal and written communication abilities, with the capacity to engage effectively with service users, families, and professionals.
- Safeguarding Awareness: Knowledge of safeguarding procedures and the ability to identify and address potential risk.
