Support young people in Bromley on their journey to adulthood! Bromley Council seeks an experienced Locum Social Worker for our Transitions / Preparation for Adulthood (PfA) Team. You'll empower young people (14-25, particularly those with SEND) to achieve their potential.

Your Role:

  • Conduct person-centered Care Act assessments and reviews.
  • Develop outcome-focused support plans promoting independence, choice, and community inclusion.
  • Work collaboratively with young people, families, carers, and multi-agency partners (education, health, housing).
  • Champion young people's aspirations for education, employment, independent living, and health.
  • Manage a caseload and maintain accurate records (experience with Liquid Logic desirable).
