A new and exciting locum opportunity has arisen within a fast-paced Hospital Discharge Team in the Stockport, Greater Manchester area. The team is made up of dedicated health and social care staff, working together to provide holistic assessment and support plans to ensure the best possible outcomes.

Responsibilities:
- You will carry out assessments under the Care Act 2014 to determine eligibility need
- Maximise rehabilitation services such as reablement & intermediate care where appropriate
- Provide assessment and appropriate interventions to aid patients in achieving optimum recovery/rehabilitation and quality of life.
- Work with patients and families addressing the impact of illness and treatment
To be considered for the Social Worker role:
- 1-3 years experience of working within a similar setting
- Diploma/Degree in Social Work
- SW England registration
- Experience of working under the Care Act and Mental Capacity Act
- Hospital experience is an advantage
