About the Role:

  • We are seeking a dedicated Social Worker to join our dynamic multi-disciplinary team, providing crucial support to individuals recovering from neurological conditions.
  • Conducting comprehensive social work assessments for individuals with neurological conditions, focusing on their social, emotional, and practical needs.
  • Developing and implementing personalised care plans in collaboration with service users, families, and other rehabilitation professionals.
  • Providing emotional support, guidance, and advocacy to service users and their families navigating the complexities of neurological rehabilitation.
