Key Responsibilities:
- Conducting comprehensive assessments under the Social Care (Self-directed Support) (Scotland) Act 2013 to identify eligibility for services.
- Investigating concerns and carrying out duties under the Adult Support and Protection (Scotland) Act 2007.
- Designing and implementing care packages, which may include home care, equipment/adaptations, or residential placement.
- Undertaking risk assessments for individuals with complex needs, challenging behaviours, or significant frailty.
- Managing the four options of SDS, allowing individuals to choose how their care budget is spent.
- Collaborating with GPs, District Nurses, and Occupational Therapists to provide integrated health and social care.
